Stream errors indicated with Qosifire

I'm using Qosifire to monitor 3 of my streams. I am inundated with the following errors every few minutes:
Wrong media sequence

Not sure where to start on diagnosing these. But they are frequent (to the point where alerting is pointless). Streams seems to play fine in JW and Radiant.
